Annual Registration Form & Parents Handbook
NB: To attend Pine Street, children must be 5 years of age and attending primary school.
All children must wear closed toed shoes to classes. Children wearing thongs or sandals will not be allowed entry.
MANDATORY ANNUAL REGISTRATION FORM AND PARENTS HANDBOOK
Each year that your child attends classes at Pine Street, you must submit an Annual Registration Form. Childrens Registration Form 2012
This is an electronic form - simply type into the grey fields, and save the document (as a Microsoft Word document - PLEASE do NOT convert to a PDF, jpeg or other format!).
Then email it to Bess bomalley@cityofsydney.nsw.gov.au

We request that you also read the Parent's Handbook for very important information about your children attending classes at Pine Street.

In person: 64 Pine Street, Chippendale from 9:30am to 5:30pm Monday to Friday (all payment methods above plus cash and eftpos)
Fees & Charges
Course fees are kept at a reasonable rate and cover tuition, administration and the use of the Centre’s equipment and materials.
Refund Conditions
The Centre makes every effort to ensure advertised courses run. However, the Centre reserves the right to alter the program either before or during the course and to cancel any program if insufficient numbers are received.
Please note that classes missed are the student’s responsibility and no refund, credit or make up class will be applicable.
